Local Rule

 

Requirements:

Required monthly for all pending small business and Subchapter V Chapter 11s, even if no activity.

Supporting documents added as attachments to the report.

Separate forms must be filed for each jointly-administered case, unless consolidated.


NOTE: Reports for the member cases(s) will be filed on the lead case.

Must be filed until:

Due date is no later than the 21st day of the month immediately following the reporting period.

 

Step-by-Step Procedures:

  1. Select Bankruptcy > Trustee/US Trustee (Bk)

  2. Enter case number and click Next

  3. Select Monthly Operating Report for Small Business (Form 425C) from event pick list and click Next

  4. Select Party from party pick list or Add/create new party,

If the case is a regular Chapter 11 case the following message will appear:

This is a Non-Small Business Chapter 11 Case. Select the Chapter 11 Monthly Operating Report or Chapter 11 Post-Confirmation Report Event.

  1. The PDF Document selection page will display.

  2. Browse, View, and Attach, click Next

  3. The following screen will appear:

  1. If Yes, the following screen will appear. If No, go to step #11

  1. If Yes, go to step #11

  2. If No, the following screen will appear. Enter member case number and click Next

  3. Enter Month Ending date and click Next

 

  1. The Final Text screen will display.  This is your final opportunity to modify the entry before submitting the filing.

  2. Verify the accuracy of the docket text.    If not correctClosedclick bankruptcy to start over/if correct
  3. Click Next to complete the filing process

 


The Notice of Electronic Filling will display. It serves as verification that the filing has been sent electronically to the court.

 

Copies of this notice and the document filed are emailed to all participants who receive electronic notification in the case. You have a period of 15 days for a one time free look at the documents.
